To coordinate services for residents who are eligible for the following services: Alternative Care, Elderly Waiver, Community Alternatives for Disabled Individuals, Brain Injury and Developmental Disability services. Assistance provided to eligible consumers that includes health risk assessment, coordinated care planning, connections to community services and identifies service providers.

Nursing Home
Kittson Memorial Healthcare Center is located in Hallock, Minnesota; with a satellite medical clinic in Karlstad, MN. The main medical campus consists of a fifteen bed hospital, medical clinic, a sixty bed nursing home (with a dedicated memory care unit), home health/public health care agency, and assisted living center. Within the campus is also a community fitness center and independent dental clinic. The Healthcare Center also operates an ambulance service that serves the community and surrounding communities.

Caring Companion Program
The Caring Companion Program is an essential community based service based on the principle that in-home care can help prevent premature nursing home placement. Caring Companions are adults 50 and older who assist seniors to achieve this goal.

Area Agency on Aging
The Land of the Dancing Sky is one of Minnesota’s seven Area Agencies on Aging. They are a focal point for helping individuals live well as they age in their communities.
